Dear Sir.

I was Mr. Shumates yesterday. who requested
me to call on you. and get some medicine for
him, he complains of a frequent Gidness, and has a bad
cough rather of the tickling Kind, when attempting to
turn in bed he is frequently seized with pain - Com
monly in his side sometimes about the hips - [(acute)] & his urine,
he requested me to mention was generally nearly as
high coloured as poke berry Juice - - When I left
him he had a fever, which was preceeded by a slight
chill, - this was probably produced by his riding two or
three miles rather earlier than he usually goes out
